Health Self-Assessment

  1. Please link to the website of Academic Information System to fill up the Health Self-Assessment.  If you need any special assistance due to any disabilities or other special conditions, please let us know so that we may offer you support services while you are studying here.

  2. Every new student should have a medical test.  If you have a medical test within 3 months and the testing items include all the items we require, the test you did is acceptable.  Even though the test is acceptable, you still have to, linking to the Academic Information System, fill up the Health Self-Assessment.  If the testing items do not include all the items we require, then you have to complete the items we require. 

  3. Please summit the medical test you did and pay NT$30 to the health test unit.

New Student Environmental Safety Training

  1. Training 1: “General Safety Training”

  2. Training 2: “Dangerous and Hazardous Chemicals Education Training”

  3. New master or doctoral program students who will make experiments in the laboratories are required to attend these trainings.

  4. Students should get 70% to pass the tests after the trainings.

Health Test

The test will charge you NT$630. Please pay it to the Health Test Unit on the Registration Day.


Please have the health test on the Registration day according to the arranged schedule. (Please check “Notices for Health Test” )


The test should be completed on the Registration day.  If you fail to do that, you have to go for a test to an appointed hospital in person.

Student Card (Campus Card)

After completing all procedures, the Registration Procedures Sheet should be submitted to the Division of Registration and then you may obtain your student ID card (Campus Card).

International Students

  1. International students, held an ARC over 4 months, must enroll the National Health Insurance in Division of General Student Affairs.

  2. Please submit two 2×2 photos, for enrolling the Taiwan National Health Insurance, and photocopies of your passport and Alien Resident Certificate (ARC).

  3. If you have not held an ARC yet, 4.5cm×3.5cm photos are required for the Resident Visa and ARC applications. (A thin half-length color photo with a glossy finish taken with a white background within the last six months that should be 4.5cm X 3.5cm with an image of the head that should not be shorter than 3.2cm or longer than 3.6cm from the top of the head to the chin, without wearing a hat or a pair of color glasses, with clear facial features not covered and identifiable, and should not be modified or composed.)

Credits Transferred or Exempted

Credits taken in prior Master or PhD programs may be transferred or exempted after the Department Course Committee’s evaluation.  Please refer to the Academic Information System/New Student Enrollment System to log on the subjects you want to transferred or exempted, download the application form and submit it, with your notarized transcript, to the Division of Registration from September 12 to September 19.

Credit Fee Payment

Notice:

  1. School starts from September 15, Monday,

  2. Registration procedures should be completed on the registration day.  If you do not register on time, we will reject your registration unless you apply for postpone, permitted by the Division of Registration, and complete within 2 weeks.

  3. You may apply for one-year retention of admission before August 31 with reasonable reasons or special conditions.

  4. Please do not pay your tuition bill if you apply for retention, suspension or dropout before the registration day.  You have to pay for your tuition bill if you apply for retention, suspension or dropout after the registration day and the tuition fee will refund to you by percentage according to related regulations.  Be sure to keep your tuition receipt for checking.

  5. You should take at least one course.  You will be dropped out if the Division of Curriculum approve you fail to take any course after the Add-and-Drop Course Selection.

  6. Penalty if Over the Tuition Payment Deadline: The tuition payment day is due on September 15.  Be sure to pay before that.  If you do not, you have to pay at the Division of Cashier and they will fine you in accordance with the Regulations.

    1. Over within 3 days: over 1 day, NT$100 will be fined, 2 days, NT$200, and 3 days, NT$300.

    2. Over 4-7 days: NT$500 will be fined.

    3. Over 8-14 days, NT$1000 will be fined.

    4. Over 15 days but not over 1/3 of the semester: NT$1500 will be fined.

    5. Over 1/3 of the semester, the Division of Registration will notify you to process the suspension procedure.  If you fail, you will be dropped out.

  7. Penalty if Over the Credit Fee Payment Deadline

    1. Over within 3 days: over 1 day, NT$100 will be fined, 2 days, NT$200 and 3 days, NT$300.

    2. Over 4-7 days: fine NT$500.

    3. Over 8 days: cancel the courses you have selected.

      • If the courses are cancelled, and with no course taken, you will be dropped out.

  8. New master or doctoral program students who will make experiments in the laboratories are required to attend “Safety Trainings” and pass a test.  If you do not, you are not allowed to enter the laboratories and make experiments.
